Open Public Records Act Request Form
The Middlesex Board of Education provides access to its records in accordance with the State of New Jersey’s Open Public Records Act, N.J.S.A 47:1A-1 et seq. Request for records should be made on the official OPRA request form and submitted to the OPRA Custodian of Records via hand delivery, mail, fax, or email.

The Custodian of Records will review all requests and provide a response writing the statutory timelines. Photocopying fees and delivery fees will be charged in accordance with N.J.S.A 47: 1A-5b.
